I.    AUTHORIZATIONS

I, the undersigned, in consideration of Republic Act No. 10173 (Data Privacy Act of 2012), hereby authorizesSt. Therese-MTC Colleges, an educational institution with business address at Iloilo, Philippines hereinafter referred to as the “School,” to process my personal data as submitted to the School, or generated as a result of the services I acquired from the School, or submitted by any third party to the School, under the following conditions:  

II. CONDITIONS OF AUTHORIZATIONS  

1. My personal data are confidential and shall only be disclosed to authorized data processors per official procedure of the school, except as may be further provided herein.

2. I am allowing transmission of my personal data by the school or any of its agents to other parties for any of the following reasons:  

2.1 My academic records may be transmitted to my parents/guardian as indicated in my school admission records for routine information and upon request of my parents/guardians;  

2.2 My personal data as expressly required may be transmitted to organizers of awards or recognizing body to which I have applied for or is nominated;

2.3 My personal data as expressly required may be transmitted to organizers of competitions to which I have applied for admission as participant;

2.4 My personal data as expressly required may be transmitted to insurance providers with which I am applying for membership or filing for claim of benefits;  

2.5 My health and medical/dental records may betransmitted upon request by a hospital or an attending doctor receiving or treating me as a patient referred or endorsed by the school; 2.6 My disciplinary records may be transmitted to the Professional Regulation Commission (PRC); Department of Education (DepEd); Commission on Higher Education (CHED); Technical Education and Skills Development Authority (TESDA); or Maritime Industry Authority (MARINA) for purposes of report submission or confirmation of disciplinary action.

III. SPECIAL AUTHORIZATIONS

1.  I am authorizing the school to post on various platforms, congratulatory messages to disseminate information on my awards, recognitions, or certifications deemed essential to promote my qualifications as a student and  program preparedness, or to promote the school’s advocacy;  

2.  I am authorizing the school to publish my graduation photograph, academic programs, academic and co-curricular awards, and organizational affiliations as I have personally submitted to its authorized publication in-charge as part of the school’s standard documentation;  

3.  I am authorizing the school to include my name, academic ranking and rating in the notices of honor roll, Dean’s list and similar documentations publicly posted;  

4.  I am authorizing the school to include my name in public notices for purposes of announcing qualification for scholarships, passing examinations, and pending submissions, compliances or deficiencies in line with grades
